  • Contact us for an identity theft packet and complete as directed
  • Request a hold on your accounts and a new debit card
  • File a police report; retain report number & officer’s name
  • Contact the Social Security Administration fraud hotline (1-800-269-0271) or visit  http://www.socialsecurity.gov/oig/guidelin.htm to report fraudulent use of your Social Security number
  • File a complaint with FTC hotline (1-877-ID-THEFT)
